Nikki's H2O Tanning welcomes new clients!  

​In accordance with state and federal regulations, all new clients must complete the New Client Questionnaire.  You may complete this questionnaire online (below) or in the salon.  You must present a valid state or federal issued photo identification upon your initial visit and have it available during subsequent visits.  

If you complete your questionnaire online, H2O Tanning will receive it when you submit and will keep it for one week.  If after a week you have not come in person with your photo identification, we will permanently delete your questionnaire and you will be asked to complete it again.

​MINORS: 

• If you are under 18 years of age, you will be required to complete the New Client Questionnaire, as well as additional forms in the presence of a certified tanning consultant. 
  
• Your parent or legal guardian must be present to sign these additional forms. 
• You BOTH must present a valid state or federally issued photo identification and if a legal guardian (other than parent), a copy of legal guardianship papers. 

Minor regulations: 

• All minors are limited to no more than 45 UV tans in any 90 days.  Spray tans are not counted in these tans unless the UV Lunar Booster is complete after the spray tan, then it counts as one of your tans.
  
• Minors who are 16-17 years of age - Your parent/legal guardian must present in a person with identification once every 90 days to sign the additional forms. 
• Minors who are under 16 years of age - Your parent/legal guardian must present in a person with identification every time you tan and is required to remain in the building during your entire tan time. 
• There will be no exceptions made to these regulations.
